by appointment only!

  • 1) Schedule visit in advance. This may be done by calling our office at 1-606-638-4586. All visitations must be scheduled.

  •  ​2) Limit number of visitors to 1 person only.

  •  ​3) Remain in designated visiting area:  

  •   a. Front Lobby    b. Resident's Room (if unable to leave bed)    c. Front Porch

  •  ​4) Wash or Sanitize Hands before & after the visit.

  •  ​5) Sign Visitor Attestation Form (this will be provided upon entry)

  • ​6) Wear a mask at all times. 

  • 7) Observe social distancing at all times. 

  • ​8) Monitor for symptoms associated with suspected COVID-19 following your visit

  • ​9) Notify The Jordan Center and Lawrence County Department of Public Health immediately of development of signs or symptoms of COVID-19 within 2 days after visiting.

the jordan center will be accepting visitors 

We understand the outstanding importance of visiting your loved ones. In order to keep our residents safe, we are accepting visitors by appointment only and allowing entry only upon passing a health screening. ALL visitors must abide by ALL protocols. 

We encourage family and friends to continue to visit via phone call, FaceTime, Skype, Text or cards.

Covid-19 announcement

We have recently completed our Covid 19 building wide testing of all staff and residents on Thursday July 9th.

 

Today, Saturday July 11th, we received our test results and we have no cases among our residents and only one case among our staff members

 

We are in close contact with this employee who is asymptomatic and in self quarantine for the next two weeks. We are also working with our local Lawrence County Health Department to ensure we are in full compliance with state and CDC guidelines.

Our residents health and protection is our top priority at The Jordan Center. Until further notice, we are only accepting visitors of those who are receiving end of life care. This new mandate is per the recent suggestion of the Census of Medicaid & Medicare and KY State Office of the Inspector General. 

We understand the importance of visiting with your loved ones, and encourage you to continue communication via phone calls, texting, emailing, Facetime, Facebook, or Skype.  

Any visitors for end of life care residents will be screened upon entry, and if showing any symptoms of an active virus, will be prohibited from entering. 

We appreciate your cooperation and understanding of our serious dedication to protecting our residents.
